An investigator from the U.S. Product Safety Commission was on-site Aug. 10, 2016, at the Greene County (Tenn.) Fairgrounds, inspecting the Ferris wheel three girls fell two days earlier.
Another non-federal investigation had earlier said that he believed a mechanical failure had caused cart No. 9 on the Ferris wheel, throwing three girls approximately 30-45 feet to the ground below.
